Request for Appeal of Offer in Compromise

Form 13711 (December 2017) is a request for an appeal of Offer in Compromise issued by the Internal Revenue Service. Taxpayers are required to provide their name, Taxpidentification number, mailing address, daytime telephone number, tax form number, tax period ended, name and mailing address of authorized representative (if any), and best time to call. Disagreements with specific items shown on the Income and Expense Table and Assets and Equity table should be identified, along with a brief reason for disagreement and supporting documentation. Taxpayers may also dispute reasons for rejection not discussed on the tables and attach supporting documents. If taxpayers disagree with the Service's analysis of economic hardship or Effective Tax Administration, they must provide an explanation with documentation. The form requires certification of both the taxpayer and authorized representative (if any) under penalties of perjury.
