Insurance Company Affidavit Without Endorsed Title

This document is an Insurance Company Affidavit for a Salvage Title or Non-Repairable Vehicle Certificate in Nevada. The affidavit includes the vehicle identification number, make, model, insurance company or salvage pool name, claim number, NV business license number, telephone number, mailing address, and proof of mailings for the first and second attempts to contact the owner. The document also requires the registered owner's information, lienholder information (if applicable), and authorization from an authorized representative. The affidavit must be submitted with a completed Application for Salvage Title or Non-Repairable Vehicle Certificate (VP-213) and a completed VIN Inspection form (VP-15). The first mailing must be sent at least 30 days after the settlement date.
